Community Support Programs - Mental Health and Disabilities

Provided by Communitas Supportive Care Society

Various programs throughout BC designed to include community inclusion for individuals with mental health conditions and developmental disabilities.
The following programs are offered:
  • Community Living Program (Life Skills): Provides training to individuals living independently in the community. Individuals learn social, personal and domestic skills. For individuals with developmental disabilities in the Fraser Valley. A referral from Community Living British Columbia (CLBC) is required to participate in this program.
  • Choices & Connections: Provides recreation and relationship opportunities for individuals in a group setting through exercise, crafts, volunteering and outings. For individuals with developmental disabilities in the Fraser Valley.
  • Community Inclusion: Provides opportunities to participate in community with staff support. For individuals with developmental disabilities in the Fraser Valley and Vancouver Island.
  • Drop-in Centres: Social opportunities, community and recreation for individuals with an acquired brain injury. For individuals with ABI in the Fraser Valley.
  • Mental Health Clubhouse: Provides opportunities for relationships, leisure, wellness support and vocational training. Activities are member-driven. For individuals with mental health conditions in the Fraser Valley.
  • Outreach: Outreach provides flexible support to people living on their own in community. For individuals with developmental disabilities in the Fraser Valley. A referral from Community Living British Columbia (CLBC) is required to participate in this program.
  • Peer Support: Provides support toward recovery through group learning, workshops, and one-on-one support from a staff member who has lived experience of mental illness. For individuals with mental health conditions in the Fraser Valley.
  • Personalized Supports Initiative: Provides individualized and personalized support to help the people we serve achieve their recreational, vocational, social, home-based and/or educational goals. For individuals with developmental disabilities and FASD in the Fraser Valley.
  • Transitional Living: Provides recovery support and life-skills training during the transition from supported housing to independent living in community. TLP also supports persons in recovery to stay independent in community. For individuals with mental health conditions in the Fraser Valley. Participants in the Transitional Living Program need a referral from Fraser Health.
  • Supported Independent Living: Provides recovery support and life-skills training to individuals living independently in the community. For individuals with mental health conditions in the Fraser Valley.
