Seniors' Community Connector - Social Prescribing Program - All Fraser Health Area Locations

For residents of the Fraser Health area over 65 on referral from a healthcare provider, navigation support to connect to services to address social isolation.
This Social Prescribing Program supports seniors to access non-clinical, community based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength based client centered approach. When a healthcare provider identifies a patient who could benefit from this program they can send a 'prescription' to the Seniors Community Connector who will support participants to access suitable local resources and supports.
