Multidisciplinary primary care centre designed to bring low-barrier, trauma-informed coordinated healthcare to the underserved populations of the Westshore and their families.
Westshore Community Health Centre (WSCHC) provides comprehensive longitudinal health care to attached patients through Family Physicians, Nurse Practitioners, Registered Nurses, and other allied health care providers. Priority populations for the Westshore Community Health Centre (WSCHC) include individuals of all ages and their families who experience barriers to health/health care due to the impact of inequity and the Social Determinants of Health (SDOH) which include: income, employment and working conditions, education/literacy, childhood experiences, built environment, lack of social supports, gender/race/class based discrimination and the intersection of same.

Catchment Area Notes:
- People who are within a priority population, who live in the above catchment area, AND who do not currently have a primary care provider will be prioritized for attachment.
- People who live outside the catchment but within the Western Communities Primary Care Network will be considered on a case-by-case basis depending on their service needs and the capacity of the Centre.
- People who live outside the Western Communities PCN are NOTeligible for attachment to the WSCHC.
