Seniors' Community Connector - Social Prescribing Program - Abbotsford

Program supports seniors to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a holistic approach.
Social prescribing is an approach to offering non-medical support that enables older adults to identify, set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local programs and services and provides regular check-ins to ensure participants are on track.

Examples of programs and services include, but are not limited to:
  • Social activities and programs 
  • Meal delivery and food security programs  
  • Transportation  
  • Physical activity and exercise programs  
  • Family and caregiver support groups  
  • Better at Home Services (housekeeping, yard maintenance, grocery shopping, etc.)  
  • Counselling 
Eligibility:
  • 65 years of age or older (For exceptions, please contact Community Connector)
  • Residing in the Abbotsford area & are community dwelling
  • Low to modest income, social isolation, low to moderate frailty, member of an underserved population, including immigrant and ethnocultural minority seniors, Indigenous elders, caregivers, 2SLGBTQIA+ seniors, and persons living with a disability)
