HIV, Hepatitis & Addictions Supports - Abbotsford Community Hub Centre (ACHC)
Addictions / Substance Use, Food, Public Health, and Sexual Health

- Provide emotional support while navigating the medical field
- Assistance with basic needs that improve an individual’s quality of life
- Medication reminders & check-ins to ensure health and safety of client’s wellbeing
- Monthly access to a high protein food bank through a satellite location of the Archway Food Bank
- Support with transportation to medical appointments, to foster understanding and ensure ongoing attendance is completed
- Managing client appointments & ensuring positive relationships/experiences are kept in the community by helping client keep appointment dates, and/or rescheduling dates when clients are unable to make set times.
- Access to peer one-on-one support
- Access to naloxone training, naloxone kits and other harm reduction supplies
- Access to shower and laundry facilities from Monday - Friday (subject to change without notice)
- Daily cup of coffee between 8:00 AM - 11:00 AM with lunch starting at 11AM - all first come, first serve
- Access to a Fraser Health on-site nurse practitioner when permitted or if an appointment is made during week days
- Assessment, referrals, access to on-site medical care, harm reduction supplies and onsite supervised consumption
- Point of care HIV testing is available on site
- Trains those in the community and other services providers on HIV, Hepatitis and STI 101
- Education and support is provided to increase each individual's knowledge in evidence-based prevention measure
- Peer support groups for those diagnosed are available monthly, with the addition of other activities planned through out the month.
- Sexual Health Clinic provides free and confidential testing, counselling, and education about sexually transmitted infections, HIV, and AIDS. To book an appointment, please call #604-702-4906
- Interagency Care Team consists of a social worker and nurse who provide support and case management to individuals who struggle with being connected to services to improve quality of life. This short-term service facilitates necessary connections for those who may be living with physical challenges, mental health, and/or addiction challenges
- Nurse Practitioners provide primary care through referral or appointment only - this is not a walk-in clinic
- Hub Pharmacy provides pharmacy services to anyone in the community
- Hub Medical provides connections to onsite addiction doctors during the week from Monday - Friday. For more information: call 778-856-0516 or email Hubmedicalcentre@gmail.com
- The Abbotsford Association for Healthy Aging coordinates and promotes activities supporting the health and well-being of seniors in the Abbotsford area
- The Ministry of Social Development and Poverty Reduction provides an onsite integration specialist on a drop-in basis to support those who have challenges accessing the Ministry office directly. For information on times and dates available, call 604-854-1101
