Provides comfort, support and end-of-life care for people in the last weeks or months of life when care cannot be managed at home.
Holmberg House is a beautiful 10-bed facility that provides dignified end-of-life care for adults from age 19.
Individuals must be registered with the Fraser Health Palliative Care Program through a referral from a physician or health care professional. The Palliative Care Team will conduct an assessment and determine eligibility for admission into Hospice.
