Community Pain Self-Management Program (FHA)
Pain Management
Provided by Fraser Health
- Topics include pain science, energy conservation, mood management, stress management, sleep wellness, nutrition, movement, self-compassion, and more.
- Participants will explore new ways of moving through gentle movement activities Individual virtual appointments as needed to assist with goal setting and assistance connecting to community resources to support ongoing pain care.
- Access to pharmacy services, pain specialists, and specialized classes is available if needed.
- An extended pain team review, including a pain specialist and pharmacist for those who need it.
- Participants will learn about community resources to support ongoing pain care, and an individualized discharge plan created with the client, along with recommendations, will be shared with their family doctor for continued support and care.
To be eligible, patient must:
- be 17 years plus a day or older
- have internet access from a computer or phone
- have the ability to participate in a group setting
- be willing to commit to 8 weeks for 2 hours
