Delta - Irene Thomas Hospice Residence

Offers 24 hour nursing care and support for individuals in the last weeks and months of life when care cannot be managed at home.
The Irene Thomas Hospice is a 10-suite residence in Delta, built to provide a home-like environment with as much privacy and comfort as possible.

Referral Instructions: Access to hospice care beds must be by referral through Fraser Health; please do not call the facility directly.
The palliative care consultation team, through discussions with the patient, family and physician/nurse practitioner, help make decisions about admission to the most appropriate hospice location.

Admission Requirement:
  • There is a palliative diagnosis and the person is living with advanced illness
  • Generally, the person is a resident in the Fraser Health catchment area
  • There must be a physician who supports a referral to a hospice and has registered them with the Palliative Care Benefits Program. Registration with the Palliative Care Benefits Program should happen as soon as a palliative diagnosis is made (with an estimated prognosis of 6 months or less), as it offers coverage for the cost of many of the palliative medications and equipment for care at home
  • The person or legal guardian consents to admission to a hospice and the hospice palliative care philosophy
  • The person lives in their own home, alone or with a family member or friend, assisted living or long term care facility
